For those interested, I did a copy/paste of the efs_extended_family_elective.txt file into a new "dynasty_elective.txt" file, replacing "extended_family" with "dynasty" as appropriate, and then created an additional section to match in the 01_title_succession_laws.txt file. Finally, I added a section to the "efs_l_english.yml" file to make the selection match in-game.
The result is a Dynasty Elective succession option, which included the great-grandchild who is the current player heir. This may not be the best way to handle it (it will get messy as the dynasty grows), but I couldn't find a way to make "extended family" include another generation.
