@Dereck Delta What? The Strong Blood gene does give benefits, not negatives. It's the Inbred gene that gives negatives. For example, gen 1 Strong blood:

If one gene is disabled, only a single roll is made. For example, if Inbred is disabled, then each pregnancy triggers just one roll for Strong Blood (10%).
In Generational mode, the chances of Inbred increase with each generation (e.g. 40% by the second generation). If that fails, a separate roll for Strong Blood (still fixed at 10%) occurs.

Like pawn 1 will not trigger a inbred or strong blood in any descendents at all (repeated testing, clearing hediff), but it can happen for pawn 2, ugh im terrible at putting stuff into words so it makes sense T^T
In short? For some the characters no matter the relation tier, its as if they are unrelated, but for others it works, but then it seems to skip a Gen, like one of them with Gen1 skipped gen2 and had a gen3 kid

That and/or it might have a issue with Big and Smalls multiple offspring/litter offspring genes, and/or early maturity

But I think it comes down to one of those things where if people don't like concept of mod or have issue with it they don't need to use it and can skip it. If people have actual issues with mod like bugs or improvement would respond to that. But feel like people trolling.
It's a good mod the way it is and thanks for making it.
@Alma_S If the relation is "duplicate" (not actual family relation label like brother/sister to simulate twins for example) then it does make sense and my mod doesn't patch that since it's not real blood family. It's a copy of a single pawn. So unfortunately I don't think my mod should support romancing a clone of oneself as that is unrelated to my mod regardless of whether that mod calls it "incest" or not (it's not).

Settings with all enabled and generational gene inheritance as well.
So I think it is likely a mod conflict? or just extreme bad luck, but it would have to be so extreme if you had over 12 births that i lean towards a mod conflict. Can you share your mod list and/or could you try without other gene-related mods?

No error popup in the log at all when the births happens
(1.5: all settings enabled + Generational, and Dynasty Lore Theme)
1. go to C:\Users\[YOUR_USERNAME]\AppData\LocalLow\Ludeon Studios\RimWorld by Ludeon Studios\Config and delete all .xml files that contain" ACI" (ACIMod, etc) or "AllowConsensualIncest"
2. do not run game yet. Resubscribe to the mod for clean install first
3. now run game and enable the mod. After restart, when you go to mod settings, you should see them just fine
Note: i don't know the exact directory path for mac/linux users or users who didn't install to the default folders, so use that as windows reference only.
Can you provide a Github link or something with your last 1.5 only version?
