if you just want to enjoy the portraits from this Mod, restart the game, then start a new game and you'll get the new portraits during the Party creation process.
If you want to change the portraits of your current Party, you should download the Modding SDK that will help you to locate the image files that need to the changed, and then you could replace them with portraits from this Mod.
So, do I need to d/l and install the whoel SDK i order to customise my party portraits?

when installed from the Workshop, the Mods will end in : <SteamFolder>\SteamApps\workshop\content\409450
you can find all the images here:

How do you change existing portraits in a current game to these ones?

you can replace your existing portrait files with the ones of your choice to update your party portraits without restarting a game.
