It's the nature of the Umbral Abyss itself, creating custom regions specifically for that layer can allow for more habitable terrain with food options as you would have custom placed everything by hand. I believe that there are some custom region mods that add to the abyss though I cannot say if they are good or not, I was also looking into a revamp for it but that may take awhile.
I'm playing with subdued umbral abyss, if that makes a difference.
From what i remember in the old Abyss cities mod, they dont build actual city in the abyss, thats reqiuring them get too far out of the way, BUT they will claim provinces through abyss gates.
And regular outposts on fixed places as in vanilla ofc.
Are they just going to build cities in abyss or will they reasonably build the special abyss outposts?
Yes, you can annex and build through an Umbral Gate both ways including the underground layer as well.

As far as I have seen it is not possible, the Underground Start entry within the 'Empire_Core.rpk' file uses the "User Map Settings Override" file type, which explicitly lists 'Spawn' and 'Override Spawn' which are set to true for the underground, however, due to the developers never intending for the Umbral Abyss to be buildable, the option to include the spawn options as a checkbox was never created for that layer.
It may indeed be possible through the use of extensive tag triggers to force it, but that is a big hypothetical.
