Only other thing I can think of would be a mod incompatibility. Are you using any other mods?
The only thing I could think of that might have hindered my achievement progress was that he had no cities at the time. But if I wait for him to get a city he just dies off from monsters.
First I've heard of it not working.
Unfortunately, I have one of the lieutenants left to slay in the current game...and the dragon happens to be it. Unlike the others he won't follow back through the portal for an easy kill.
Mods that are already active on a save, can be updated, and the changes WILL take effect. That's actually how I tested this mod. I had a save with this mod active, set up so I could kill him next turn. Everytime I would change something to fix it, I would load the save and kill him. If I didn't get the achievement, it didn't work. Eventually, I got it.
Though there are probably some changes that won't work, like level-up trees or something.
