Yeah, i'd say so, could be a mod that's not playing nice with this one.
Do you have anything else that changes companions at all?
Try re-jigging your load order, or using LOOT/BOSS to order it for you, it might be able to spot a conflict
Hmm, i'm not so sure there bud, I've certainly never seen that with just my mod alone. It certainly could be, but I doubt it.
You're definitely correct there, almost all mods work better on a new game, particularly any that have an effect on world spaces or stats; things that improve follower level for example need to either have a fresh save or something built in that re-loads the follower's core files in order to update their data.
Here, however, a new game isn't needed, as all this mod really does is change a flag in the followers files that dictates how the character handles death; basically it's a tick box, and is such a small change that the game doesn't need to reload anything for the follower.
If I can direct your attention to the description my good sir, this is explained:
"NOTE: Followers will still kneel down and regen their health, enemies will still aggro to you when they do this and they will still get back up a few seconds later, however, with this change once your follower starts attacking again and retakes aggro they will die (or on the second "standup" or third), they WILL eventually die unless you pull the enemies away from them. I've tested this on all altered followers, enemies will use finishing moves on them, dragons will eat them, etc."
Console commands will always work, if you're particularly cut up about the loss; personally I always just roll with the deaths of my companions and work it into whatever i'm doing, getting revenge, etc, I find that creates a lot more immersion and investment.

You can always bring her back, if you still have the body; the console commands resurrect and resurrect 1 work like a charm.
Resurrect basically recreates her, but resurrect 1 just revives her (it's the better one)
You can also try and "Revive" her using certain unscrupulous conjuration spells...

I'm pretty sure Derkeethus should be fine, it's possible that because of his quest-ish tags I left him, but i'm 99% sure I didn't.
Thus I don't really know how to fix it, sorry =/
Okay, it could be your Khajiit followers that are messing it up, but don't worry, you can still keep them, just gotta fiddle with the load order.
Basically (i'm assuming you know about load orders and whatnot), make sure my mod is BELOW the Khajiit mods in your load order.
Alternatively, try disabling the two khajiit mods temporarily and testing to see if that works.
I've got a few other things to ask and try, but try those two first and let me know how it goes
When you say "Her" who do you mean?
Can't think of why it wouldn't be working unless you have other mods that change or alter followers in some way, like custom followers, multiple followers at once, that sort of thing
I'll certainly put it on a "to do" list, but i'd need to get the time first, so I won't promise much as of yet =)
She should be killable, certainly; is sne kneeling down when her health depletes?
The only things that I can think of that would have made her unkillable are if you've married her, or if there is some kind of quest involving her that is active; it could also be caused by any other mod that modifies Mjoll, or followers in general, like Ultimate Follower Overhaul.
Looking back at how I've written the description, it's a little confusing, it looks like I've said "All the hirelings" and then gone on to list the names of those hirelings, but what I meant was to have hirelings as its own category (Because I figured it'd be easier than naming all of them)
Now you'll thin twice before leaving them alone.
The only drawback of this mod it that is set of the compagnons' name in english sinc eit is an english mod (I am not english or amercican).
BUT it is not such a problem since most of the compagnons' name in english or any other languages are the very same.
Only Mjoll the liones changed in my game, that is all, so it is worse it, great mod, THX !!!
Personally I like it, adds a nice RP element
Basically, It shouldn't matter which mod is at the bottom as long as it doesn't affect followers (in this case), if it does, have my mod underneath it, and if not then don't worry, just have them both as the last 2 mods in your list.
