To your question: I usually use it on Very Hard (mostly, because I don't like the Realism Battles of Legendary). But because those are massive upkeep decreases for the AI, they will field significantly more armies. Especially the early game might be nearly impossible if you make a few strong enemies and can't save before battle. However it should be quite playable, even on Legendary, if you pick a faction which won't let you drown in an ocean of enemies early on (like Tyrion for example).
However, it simply buffs AI's income, costs and growth what will lead to more and bigger armies but also to better developed cities. The exact changes are in the description.
