you can't. its in a vpk-file that i made.
this is how the steam workshop works with l4d2, you download vpk files which contain the moded files.
and it just instantly works in the game.

I wish, but i'm not a good mapper, i did one unreleased Killing Floor map, and and a few maps for Doom 1/2 and HL1 thats all.

And i'm not sure that i can import Duke Nukem maps to this game, not with out help.

I'll see what i can do, but some people don't like it loud(er), they say they can't concentrate on other sounds, but i'll try to find the perfect balance :)
