Improved Colonial Nation Formation and Dynamic Borders

Description
This mod changes the way colonial nations are created in order to have more consistent nations (without difirent bits that are almost half a continent apart) and have a bit less awkward names (like Florida in Boston).

There are more and smaller colonial regions, nations are created like they normally would. If a province right next to a colonial nation is colonised it will become part of that colonial nation, no matter what region it's in.
Basically, the nations will adapt the name of where the capital is and will only create a new one if a new part of the continent, separated from any other colony is colonised.

Borders between same-country colonies can happen if those colonies start separated and expand to border each other.
