2 - map from the source version of Half-Life Absolute Zero (addon in the Garry's Mod workshop).
3, 4 and 5 - maps from Half-Life Blue Shift, an unofficial version. There is an addon on the gamebanana website and in the Garry's Mod workshop.
You can find and install these addons and unpack them in SFM.
There is a possibility that Half-Life: Blue Shift Source can replace some files, for example LD versions of models on LD-HD (The model looks outdated, but bones and some animations will be replaced from the HD model).
As for HL:AZ source, it seems to be in Moddb Half-Life Source or Moddb Garry's Mod, but if you do not have Garry's Mod.
