Check school desks and office desks and one of the art supply stores in Louisville. If the bic pen doesn't appear at all in those loot tables, then something is breaking the mod.
If it DOES appear in those loot tables, it might be that you've been extremely unlucky RNG-wise.
I wasn't aware More Loot Settings has problems in multiplayer - currently haven't had an issues
the only other mod we use that affects loot tables is 75% less loot, since More Loot Settings doesnt really work in multiplayer.

The mod itself doesn't do very much, but if you are using mods modifying how the procedural distribution tables work, that might be an explanation.
I suggest running the game in debug mode and using the LootZed option (it's under cheats) and then check the loot tables for school desks or office desks. If the pen does not appear at all, there is something strange going on caused by other mods.
