The permadeath wasn't forced in, and the random items don't really make for a poor design decision. You might not like them, but they are part of the genre. I would advise that if you see a game with the tag "Rogue-like", you should probably stay away.
The difficulty meter is there basically to keep the game challanging while you gain more power through items. It's up to you to find out how to keep up with the meter so the difficulty will not overwhelm you.
Items may be random, but with enough skill and appropriate tactics the chances of failing are low (unless on monsoon).
Until you get the hang of it you should focus on completing levels fast. Rush to portal and don't stop, don't waste time, kill monsters while constantly moving, don't try to collect all items, get the ones that you find along the way and while backtracking to kill remaining monsters after killing the boss.
