Sorry to tell you but on the mods side the witch is fine.
Maybe an interaction with a different mod is causing this or your mod version is somehow corrupted. You could try to desubcribe and subscribe again.
If this didn´t works look through your other mods for these who do changes to the races (forms).
Btw, is it happening to one form specifically or for all?
