1.) The terrain is completely flat even outside of the towns and villages, some microterrain outside of towns and villages would make the terrain look more natural.
Might be something thats already planned for later after placing is finished, but might be worth mentioning nontheless.
2.) The runway looks unnaturally stretched out, especially for a smaller terrain like this, , something ca. half the size would look more appropriate.
3.) A playground at the areas with bigger chunks of multi story building houses, maybe some gardening/gras areas to make it look nicer to live at, which also helps to make some areas look more detailed and different to other.
As this terrain uses RHETERRACORE mod there are a nice bunch of new objects to play with to achieve that when actually integrated.
4.) The island has no power station unless i overlooked it Together with some trafos nearby it, it creates a different kind of mini location and also a point of interest of missions.
5.) As the terrain has no military installation it might be interestng to add a compound that works as police station, including garages/places for police cars, parking space for officers private cars, a wall/fence that protects the compound,
1) Yes planned
2) https://imgur.com/a/AEq1AtH
~ 45m
I know but after resizing the satmap roads and other textures will be too small
3) Small terrains are good choices to add more details and I am working on custom objects, ...
4) Power station:

More details will be added
5) Of course. This island is heavily militarized and there will be trenches around the island (you can see them https://www.google.com/maps/@25.877085,55.0320044,8407m/data=!3m1!1e3).
"A compound that works as police station, including garages/places for police cars, parking space for officers private cars, a wall/fence that protects the compound" nice idea
I was refering to the length of the runway not its width. No idea if that makes any difference to your mentioned issue with the satmap.
From a mission maker perspective the runway makes basically any operation from the S of the island unpractical. Especially in PvP based missions you just need a few players with binos at the runway and some people at both ends of the runway and you basically made any stealth operation to move further N pretty much impossible.
Not so much of a problem in Coop missions because of the limited detection range of AI, but from a immersion point of view it does not work well to suddenly cross such a relative big area, you usually would probably move around it, which is basically not possible as the runway pretty much goes across the whole island beside a small stripe of island on both ends.
This makes also the replayability a bit more difficult as usually you would be able to try different routes by making use of the microterrain to find better ways to your destination.
I would not consider that a big issue, but i think its something that could improve the terrains useability for missions.
Hard to recognize at this point, especially if you are not aware of this type of small power station building. I was looking for the much bigger and much more vislble version (Land_DPP_01_MainFactory_F) and some trafo stations.
I noticed that you were using the CUP version of the building, there is also a Arma3 version which has the advantage of being enterable. (Land_PowerStation_01_F)
Good to know, i thought at this stage most of any major military installations would be already present.
