While there's no errors thrown or anything, it seems the only way to off-hand a weapon that has been equipped as a sidearm is to drop it and then re-equip it from the ground. Not sure if I am misremembering, or just have something misconfigured or what, but I swear I remember a little button integrated into the Simple Sidearms UI to off-hand an eligible weapon.
