Mate, this thing is over 6 years old.
It's been almost 5 years since I last worked on this and I don't play GMod anymore.
The intended use case was dedicated servers running TTT, where you'd just configure CVars in a config file. So I'm not familiar with persisting CVars on a local client.
It should show up in the traitor buy menu. Note that this weapon won't show up in sandbox mode, just Trouble in Terrorist Town.

You did nothing wrong, this was a bug in the addon. I just fixed it, thanks for the report. Just restart your server and the update should get loaded.

Can you eloborate on how frequent / reliable it occurs on your end and what steps you take to cause it to occur?
Basically, whenever a victim gets poisoned they get the damage for X ticks and X intervals - and that's OK.
Though, the victim may instantly die when the shooter kills another person completely unrelated to the victim - and the cause of death of the victim is the same as the other person shot by the shooter.
If you need further clarificatin, I'm available.
In this case, I actually found a few critical issues that I believe are fixed now.
