There was another one that did, but it appears to have been removed.
Not sure then. A disgraceful number of mods forget to factor in body/skin groups & colours, so I'm not able to figure out which mod in particular could be causing it without browsing your subscription list.
My shortlist of things I'd suggest to check are mods like player menus, PAC or Outfitter, or persistent mods for models/weapons.
I'd also recommend checking your model in third person prior to death to confirm the reset. Use the camera tool and get a look at your model at the points before and at death.

Ty the Tazmanian Tiger was on Nintendo 64, so probably not.
I wouldn't know. I played Halo 3, not Reach.
Try subbing to the thing I just added as required and see if that fixes it.
The bodygroups included were the only ones included on the original model and I A) don't know how to port from Reach and B) can't rig for shit.
The scripts are literally less than a megabyte.

Check the console and see if it says anything about a .vmt error in red text.
Models have nothing to do with lag because models are cached.
