My idea would be that you only need Rome to establish it. However, that would only change the name and result in a debuff if you establish it too early (the strength of the debuff depends on the size of the territory you control).
To remove it, you would have to conquer Roman territory and prove that you're neither a fool nor a madman.
Cores would have to be unlocked through decisions that take at least a year and tie up enormous resources during that time.
to form Outremer, you either need to release and play as the Papal State in Italy or form it through Mandate of Palestine. (if you go through Palestine, i recommend taking the Holy State path, it’s more historical that way.)
i’ll be keeping the requirements strict for the more historically accurate formables, since that’s part of the challenge. but for some of the minor nations I’ve already made the conditions a bit easier so they’re more accessible. for example, certain Middle Eastern, South America, Asia and Balkan nations.
