They both reference the same character, but different depictions (Coolchou's Metal Gear Solid or Metal Gear Solid: Twin Snakes, this one Metal Gear Solid 2: Sons of Liberty or Metal Gear Solid 2: Substance).
Let the better referential attire win, I say.
Also, Ludwig is Medic's real name. Not sure why it's called Ludvig here, but it's fine. Each cosmetic are pretty different from the other, even if it seems to be referencing the same material.
Also the other set has the jacket be compatible with shirt items (as it's just a jacket on top of Medic's usual attire), here, it seems to be a shirt item itself.
Hair is nearly the same, but here it's more "oily", while the other is more messy.
I think both sets can co-exist with each other.
