For me it was also not showing the head texture but after I installed and mounted HL2 EP1 it started working.

@nuhuh No. Try verifying your game files or mounting Entropy : Zero 2/Half-Life 2
Head textures should be included with the gmod though so this case is weird

does the scar zero need a mount because the back of his head is missing textures

that is probably because you have too many addons. i just killed the npc in game and nothing happened
