This map is really creepy because of the ambiance, like someone sharply breathing through a ventilator which really adds with the hospice theme. It's like someone's final moments

you can easily make one yourself via nav_edit 1 and nav_generate and then nav_save. (Just make sure sv_cheats is set to 1 first)

No, I did not take anything from the Slender Fortress version. This map was made from scratch.
