the default configuration is to spawn at either the insert location, FOB (if not inserting), or near the average group location. if there are no valid respawns, you should still get the respawn screen, although with a notice that respawn is disabled.
did you disable respawn or respawn positions in mission settings? the default values are "20 seconds", "Both".
yes, although i'm not sure why you would. on death, you should have the option to respawn at both the insert location (or FOB) or a somewhat buggy implementation that averages the positions of the squad and you respawn as a new unit directly (you can keep your inventory too if you have that enabled in ACE).
you can edit the mission's description.ext and change respawn = 3; to respawn = 5; to "take over" AI units and enable the teamswitch menu (default "U"), but that might have strange behavior.
glad you found it while i was writing out my response. the stamina option is meant to always be enabled to feed data to ACE Advanced Fatigue. if that data link is broken, the fatigue functions in ACE enter a loop that never exits, which causes animations to never complete, freezing AI in place (and the player too, typically).
the option is there for people that have a customized ACE version that has ACE Advanced Fatigue removed or disabled and want to use vanilla A3 stamina system or disable stamina completely.
if you've already tried running just CBA, ACE3, and then the terrain itself (no other addons), it could be some oddities in profile or settings for DRO or ACE.
if you are using ACE Medical, be sure that "Revive" is disabled in the DRO options, and if you are using ACE Advanced Fatigue that "Stamina" is enabled. you can also click the "Reset Default Options" button on the information tab when DRO first launches a mission (the main menu where you pick your mission options).
while it'd be pretty neat to do, there are no set locations on the "Arctic" map for the scripts to use to generate a mission, so something like DRO or DCO will not work without heavy modification bordering on a complete re-write to use pre-placed markers or something.
