My advice is to either 1) manually remove this mod and to let Steam re-download it (see older comments for Windows instructions, Linux will require a tiny bit more effort to find the folder that TBOI is installed to, but the steps will be similar) or 2) narrow the crash down to an incompatibility with this mod and a specific other mod so that I can reproduce the problem.
Without the ability to reproduce a bug for myself, I'm not able to produce a fix. The only thing I can say definitively right now is that this mod does work properly on my own devices, including my steam deck, and is not crashing for me.
As for crashing with the R key, I tested that also and couldn't reproduce it. An R key restart doesn't trigger the mod, but also doesn't crash the game for me.
1. Open MCM.
2. Go to TR Start -> Settings -> Allow Starting Curses and make sure this is set to "ON".
3. Go to TR Start -> Curses and make sure everything is set to "OFF".
I realize that's not very intuitive, but the explanation is that when "Allow Starting Curses" is "OFF", you'll never start with a curse regardless of what options are set on the Curses page. If "Allow Starting Curses" is "ON", then you're guaranteed to get at least one of the "ON" curses from the Curses page. If none of the curses are "ON", then the mod simply doesn't factor curses in when deciding to reroll your start, effectively giving you vanilla behavior.
Let me know if that works for you.
2. You can open "Documents\My Games\Binding of Isaac Repentance+\options.ini" and change EnableDebugConsole=0 to EnableDebugConsole=1. Once you're in-game, you can press tilde (~) and confirm that there's output that says something like "Treasure Room Start v1.15.0 was loaded".
3. The only other piece of advice I can offer is to manually delete the mod from your game folder and let steam redownload it.
If you do all 3, things should be fixed.
Thanks.

Gracias.
Deleted the .dat file, and now it's v1.15.0. Seems to be working now.
Another thing you could try is navigating to "C:\Program Files (x86)\Steam\steamapps\common\The Binding of Isaac Rebirth\data\treasure room start\" and deleting any .dat files that you see there. That's where TRS stores its MCM settings, and it might help to wipe them out (effectively resetting the options back to to defaults).
