Join my server (IP: 91.185.185.20:28185), type "nominate" in chat, choose this map, then type "rtv" and choose it again. Enjoy...

I think that map only works on dedicated/hosted (not in-game) server. Don't know why.

Is that text from your server console? Did you run map with custom mode? This is strange problem cause this map works fine on my server. You can test it here: 91.185.185.20:28185

You must hit it with knife. Knives sometimes don't appear, but usually you get one by climbing on ladder.
