Make sure the skull becomes invisible whenever a Spy who "wears" it cloaks or disguises. Unless the target also has such a skull.

Cool concept. Model looks nice, too. (I will ignore that the teeth are only on the front of the jaws, lacking molars. Maybe those fell out.)
