Some player experiencing that too...appearently maybe because they haven't finished yet the story maps.Especially the sacrifices map. Just make a folder named namvet in dlc1 and copy the files there.

This only works for Bill survivor which named namvet in your l4d2 folders, do not replace to other than Bill/namvet folder, it will cause no sound to those survivors, but you can always to revert it back with verify files integrity in steam options, also do not mistaken the specific folders.Ex: dlc1&2 only for navmet folder dlc 1 and dlc 2, not for dlc 3

After I replaced all the files and ran snd_rebuildaudiocache, after it finished and I relaunched the game, none of the survivors' voices work. Was there an extra step I missed?

Hmm never happened to me before, well yea you could try verify cache, but all ur changed files, such as sound mods, would be changed and steam gonna redownload all of it again. So you need to change it from 0 again.

it is designed for Bill, to be honest yea it would work, IF only you replaced the files on the coach folder which is "voice\Coach" folder, not "namvet" folder. But some of the voice might be error, cause various reasons such as different length between Bill's file and Coach's file, hence you might don't wanna do that
