The mod's outdated for the current game version, not "fake". This fact should be obvious.

add inside of the .zip of the mod at the end of defines.lua in the "common" folder just the lines in bold , the remaining text is just for reference

This is entirely possible, but I have no idea as my modding experience with CK2 is very limited.
No. This mod is one based on the randomize world option provided with Holy Fury. This allows you to either have completely new cultures and religions, or use the real ones. The map itself is not really playable without this, so nothing by default is "where it should be" per se.

all of your steam downloads should be in ( program files 86, Steam, steamapps, workshop, downloads) by default.
If its not showing up in the mod menu, that is an issue on your end as it should be there. Try uninstalling and reinstalling the mod.
I did this with an updated version of the ck2 map generator, if I can find it again I'll link it in the description.
