The 4 tiles limits is for building city close to an opponent/city state cities, I think Mdk25 extended the minimal tiles between your own cities.

I just played and could not have cities within 4 titles

Nope, default is 2 (this is what allows the horrible city spam by the AI).

I thought they already had to be 4 or so apart.
