There's a KF2 beta build going on at the moment which is likely what is causing these issues. I'll keep an eye on the problem to see if it persists after the beta goes public, there's little else I can do about it at the moment.

Also, I am not sure if it's somehow possible, but would it be possible to make crawlers spawn in the little areas above the ceiling? So that destroying the ceilling actually is a disadvantage, because if you do so, new spawn points are being generated.

I'll have the skylight in the office fixed (damn shotgun jumpers :D) and I'll take a look at the ladders and see if I can make them more zed-friendly.
I've been very diligent with movement optimization (insanely boring work, but any serious mapper should be doing this) to avoid players getting stuck in geometry, so if I've missed some glaring spots I'd be interested in knowing. Bumping into things that are obviously there can be unfortunate, but remember much of it (such as desks) can be broken by players with regular guns (unlike in the official maps), so if you get locked in it might be faster to shoot the desks instead of the zeds.
I do like the atmosphere as well.
