minor fixes to some decisions

fixed and improved the maintenance event

- somewhat simplified the mod, removing the special title that was prone to bugs and oversights
- integrated my other mod making improvements to chaplain appointments

- some fixes
- removed special excommunication request (it was bugged, and I don't think it made much sense)
- lowered some of the negative impact from being an autonomous church
- now you get a strong hook for newly placed head of church, guaranteeing full vassal contribution
- added an interaction to check other autonomous churches of your religion
