Following this simple guide you'll be able to play the game on the newest Windows versions (8.1, 10 + x64) with no stuttering issues and on any resolution without the image being stretched out.

How to get the game working perfectly on modern systems

Right-click on the game in your Steam Library and open "Properties".
Switch to the tab named local files and click “Browse local files”.
Run the file “Config.exe” and enable “32 Bit Color Depth”, the rest should be left as it is.
Download the file from here[dege.freeweb.hu] and unpack it.
Move the extracted files into your game folder (it should still be open, otherwise reopen it).
Run the file “dgVoodooSetup.exe”, click “Add” on the top-right and select your Sacred game folder (*your Steam install location*\steamapps\common\Sacred Gold).
Under “Appearance” set the “Scaling mode” to “Centered” (this will make sure that the game won’t be stretched and kept in the intended 4:3 aspect ratio).
Switch to the tab named “DirectX” and under “Videocard” set the “VRAM” to the highest setting that your graphics card can support (for best performance).
Under “Resolution” select the resolution you wish to play the game with.
Under “Miscellanneus” disable “dgVoodoo Watermark”.
Hit “Apply” and afterwards “OK” at the bottom.

I highly suggest you to leave texture filtering and antialiasing disabled because with either enabled you get a lot of graphical artifacts in the form of lines across the screen.

do i have to start the game in a specific way? because the fix doesn't seem to do anything for me...

When u have multiple screens, U also have to select your graphics card as the adapter used and your first monitor in dgvoodo. Otherwise the game won't start.

так эта программа просто растягивает его не пропорционально, выглядит так себе, в родном 4\3 играть приятнее намного

I run the file “Config.exe” and enable “32 Bit Color Depth”, along with "Video Compatibility Mode", and inside the Local Files at the "Sacred" application-> right click -> properties -> Compatibility -> set at Windows Vista service pack 2. It worked for me.

SandinavianEarl - you need to play with them on Ancaria until at least level 27 or 29 (I forgot which, its been years), and then export them. Once exported, you can start Underworld with them.
I know it's been months since they commented, but maybe it helps someone.&
