I dunno what's up with the face but I know why the eyes are so fucked up.
If you look at the actual boss fight with him in game, his eyes are always glowing orange.
This means that you never really see his eyes because they're covered up by a particle effect the entire time.
The only thing I can think of is if you modify the model so that it has the regular mercer head by doing a little bit of editing and combining.
