Z-City is built around Homigrad, its original concept.
Z-City features a wide variety of weapons, melees, and grenades ready for use, as well as its own bullet and ragdoll systems. Main features include:

Realistic damage system

Dynamic bone and organ simulation

External and internal bleeding

Pain, fear, adrenaline, and shock affecting movement and consciousness

limb fractures with permanent consequences

Advanced bullet system with caliber, and penetration calculation

CPR and functional survival mechanics

Memory system during critical states

Emergent ragdoll physics with interactive VFX

Also, spectators cannot talk in dead chat. Found the issue aswell.

lines 251-258 (Player Death hook)
lines 240-248 (Player Getup hook)
and line 705

the logic seems backwards - the PlayerStartVoice hook should allow spectators to talk to each other but they cant communicate

the problem is in the HG_CalcView hook at line 65-125 in the cl_init, i saw some bugs like

the hook is processing spectator view even when it shouldnt
player models arent being properly hidden during certain states
the bone scaling isnt being reset properly
