To get this achievement you have to push the Wasteland Shepherd off the map, the Wasteland Shepherd is the floating skull that gives you the tutorial and he will be missing in your map if you randomise it so use the default map for this.

As you spawn in next to the king, the ram's head skull will be floating to the left of you, don't bother talking to him and if you do, make sure you don't say the top option "Hello! I think we've already met." As this will skip all the tutorials and make him disappear resulting in you not being able to push him off the map anymore.

All you need to do is keeping walking into him, pushing him all the way over to the edge of the map, as you push him off he will fall to the death barrier below the map and die, giving you the secret achievement.
