Subscribe, then head to;
C:\Program Files (x86)\Steam\steamapps\workshop\content\546560\3464138514

Use GCFScape to open the .vpk and extract the folders to:
C:\Program Files (x86)\Steam\steamapps\common\Half-Life Alyx\game\hlvr_addons\(your addon here)

you should put an [S2FM] tag so everybody knows its for S2FM, i almost thought this was a model replacement.
