I've just tested it now with 1.16.2 and it seems to work completely as intended. I haven't run into any of the issues mentioned in those comments, so I don't really know what to try and fix.
I have, however, not tested it with 1.17.x, but I'm not touching that version anyway until Paradox gets more fixes in. So just for now, all my mods are not guaranteed to work with 1.17.x, and I will return to check and update everything when it's more stable.

As for your question, you can't create entirely new duchies with this mod, but you can reassign counties to different duchy titles and rename the duchies as needed.

It's the itself. You don't need the original. The mod is already made up of very few files, so a fixed version basically just has to be a copy.
