i looked in my addons folder and found "Hidden addon", clicked and searched the ID of the addon in my addons folder, found this map, and decided to re-upload it.

i don't know the original creator, the description, or why it was taken down.

there is one jumpscare in the map with the figure standing in the room with the chair
