It can do either one really. But for this one they should be spawning in using the bodygroups so you should get what you see in the picture with them spawning in different outfits.

how does this work, exactly? does it randomize an npc model's bodygroups upon spawning them, or does it just spawn repeatedly npcs with the same default outfit?

I tried to do that but only the females worked so unfortunately I don't think you'll be seeing replacements or npcs for those anytime soon unless I can find a way.
