This is a completely handwritten guide. Any item / collectable you need can be found by using CTRL + F and entering what you're looking for in the text box.

Mods are available for this. I've created a complete section for modding the game below.

Use can use the interactive map to find seeker tokens, dungeons, golden trove beetles, and more: https://mapgenie.io/dragons-dogma-2/maps/world

Tips for achievement hunting
Remember where you find your first seeker token as you'll need to return to that location later. There is a mod that marks its location on your map if you're having trouble finding it and a mod for marking all token on your map. The links are found in the Modding Tutorial section below.

Pawns who say they have an act for combining items will consume important items from your inventory. Stay far away from them if you're someone who likes hoarding items like me.

Place a portcrystal in Battahl as there isn't one in this main city for some reason??????? Devs????

For getting all achievements in a single run, you can follow the "Story Achievements" and "Post Unmoored World Achievements" sections. The achievements are listed in the order you will obtain them. Then you only need to keep an eye out for the missable achievements:
Master of the Maisters - Failing a quest could prevent you from getting some techniques.
Are We There Yet - Finishing the quest before riding the Phantom Oxcart causes you to miss this.
The Barbecue-Maister - Complete before going to the Unmoored World.
Full Marks - Can be failed if wrong answers are given to the Sphinx.
Reaper's Scorn - You must defeat the Sphinx to get the Eternal Wakestone.
Story Achievements
First Taste of Freedom Escaped the bonds of slavery. Story related, cannot be missed.

Arisen Regained your memories of receiving the Arisen's charge. Story related, cannot be missed.

Seat of the Proxy Arrived in Vernworth. Story related, cannot be missed.

Nobles' Night Out Attended a palace masquerade in formal raiment. Missable! During the main quest "The Stolen Throne", talk to Captain Brant and pick his dialogue option "Tell me of the masquerade". He will give you a mask called Eventide Mask. Go to the Comptroller's House south of the Vernworth portcrystal, in the Noble Quarter. Upstairs you will find the Courtly Tunic & Courtly Breaches, equip these and the mask. Go to Vernworth castle (south from your current location), enter the masquerade room at waypoint 1 during the night and the achievement will unlock. There's a masquerade every other night, so rest at a bench to skip a day if there isn't one that night.

Myrmecoleon Delights Employed the services of the rose chateau. Story related, cannot be missed.

I'm In Used illicit means to pass through the gate at the border. Missable! Put the "Beastren Border Entry Permit" and/or the "Battahl Residence Permit" in storage if you have them. Go to the oxcart station in west Vernworth to fast travel to the Border Checkpoint Town. Make an inn save and rest until morning. Head back to where you exited the oxcart and look for a carriage. If it didn't spawn, rest at the bench by the inn and try again (not the bench near the oxcart station). Get in the carriage and wait until it passes through the gates and the achievement will unlock. This will make enemies hostile and prevents you from returning. Load your inn save and proceed through the gates using either permit so NPCs don't turn hostile.

Across the Border Passed through the gate at the border. Unlocked with the "I'm In" achievement.

Roost of the Dragon Reached Dragonsbreath Tower. The main story will take you here unless you already have 15 wyrmslife crystals. You'll come here after talking to Luz about finding lesser dragons. This is southwest of Battahl.

Back Where It All Began Returned to Agamen Volcanic Island. Story related, cannot be missed.

Gigantus, I Hardly Knew Ye Defeated the gigantus in a short span of time. Missable! The Gigantus is a main story boss you encounter during the main story quest "The Guardian Gigantus". To quickly defeat it, purchase an unmaking arrow from the Dragonforged or get one from the Sphinx's riddle. You'll need to use the Archer Vocation to equip the arrow. Shoot Gigantus while it is still in the water and all the crystals will fall out. It will continue walking along the path up to the first ballista on the mountainside. Shoot it a few times with the ballista and it will die granting the achievement.

Important: Make an inn save after beating Gigantus and entering the slave camp you began the game at. You'll want to complete as much of the game and as many achievements as you can before continuing. Many quests will fail/expire when beating the final boss. Place a portcrystal at the slave camp so you don't have to walk back here. When you've wrapped everything up, follow the steps in the next achievement to continue.

Peace Became Sovran of Vernworth. Missable! Make an inn save at the slave camp. Proceed through the camp, up the lift, and to the final boss. Defeat it and sit on the throne and you'll get the "Peace" achievement. DO NOT speak to the ghost. Return to the main menu and load the inn save. Continue to the next section of the guide below for getting to the Unmoored World.
Post Unmoored World Achievements
The achievements in this section should be obtained in the order I have listed them. The world will shrink as you play so you should focus on objectives near the edges of the map first. You have about 14 game days to complete everything. This is quite a bit of time so don't feel too rushed.

Dragon's Dogma 2 Witnessed the unmoored world. Proceed to the final boss again, walk up its back while it flies to the arena, and use the Empowered Godsbane Blade on yourself when your heart glows. The dragon will fall from the sky and you'll enter the Unmoored World without needing to fight the final boss again. Note: You can get 100 Wyrmslife Crystals if you defeat the dragon normally. So defeat the dragon and speak to the ghost two times at the ceremony. While it's flying to the arena again, walk up its back until your heart glows and use the Empowered Godsbane Blade on yourself. You'll go to the Unmoored World and have the 100 crystals for defeating the boss normally. This will help with purchasing the Dragon's Dogma weapon for another achievement.

Dragon's Dogma Obtained Dragon's Dogma. Missable! Obtained from the Dragonforged for 110 wyrmslife crystals after making it to the Unmoored World.

I, Talos Helped the gigantus walk again. Missable! After meeting up with your pawn, head to where you defeated Gigantus. A cutscene will play when you approach it and you'll get the achievement. If Gigantus fell into the lava you will not be able to get this achievement. You won't need to worry about this if you're following the guide and defeated him quickly.

The Hero Overcame all the trials of the unmoored world. Missable! Complete the three pillars of red energy marked on your map. Walk up to them and use the Godsbane Blade to initiate a battle. If Gigantus fell into the lava, you'll have to complete an extra two beams for the achievement.

The Guardian Led the people to safety in the unmoored world. Missable! You need to evacuate Vernworth, Bakbahtal, and Sacred Arbour by completing a side quest in each. Vernworth: Speak to Brant at the inn and he'll give you the quest "The Regentkin's Resolve". Speak to Sven in the castle and he'll give you two tasks to complete, gathering oxcarts and dealing with his mother. Simply aim your weapon at the oxcart owner and he'll run away. For Sven's mother, take the Ornate box in her room to Sven if you helped him before. If not, pick up his mother and carry her to him. Completing this will cause the people of Vernworth to evacuate. Bakbattahl: A beastren, Menella, will speak with you along the main road to the Battahl Palace. You'll be given the "Civil Unrest" quest which involves solving some issues within the city. Follow the markers on the map to complete the quest and the people of Battahl will evacuate. Sacred Arbour: You'll need a pawn with the "Woodland Wordsmith" specialization to translate Elvish for you. You'll also need a rotten fish and apple. Speak to Doireann near the Arborheart (big tree) and she'll give you the quest "The Ailing Arborheart". Head to the Battahl smithy to complete the quest and get another quest "Out of the Forest, Into the Forge". Follow the objectives, complete the quest, and return to Sacred Arbour to unlock the final quest "Wandering Roots". Complete this quest and the Elves will evacuate. The marker to plant the tree is directly in the center of the yellow circle. Note: It might be possible to quickly evacuate Sacred Arbour by talking to the Elven leader Taliesin multiple times. Speak with his son and daughter first then speak to him. He won't want to evacuate at first but will give in after speaking with him four times or so. The achievement unlocks after all three cities evacuate.

Closure Experienced the end of the cycle. Story related, cannot be missed.

Modding Tutorial
Some might want a completely vanilla first playthough but mods are available. I'd recommend an item weight to 0 mod and the "Crazy's Shop" mod. The 0 item weight so you can carry anything you want and the shop mod so you can purchase portcrystals and ferrystones. There are only TWO static teleport stones until you beat the final boss and it takes a ferrystone to use them. Using this mod will will literally save you 10+ hours of walking between cities. Search the "Popular(ALL TIME)" tab to view the most popular mods.

You'll need the "Fluffy Mod Manager" and "REFramework" mods to install other mods.

Fluffy's Mod Manager: https://www.nexusmods.com/dragonsdogma2/mods/1

To install Fluffy's Mod Manager, click on the link above, go to "Files", and click "Manual Download"

Create a folder on your desktop (right click desktop -> New -> Folder) and name it "Mods". Then open File Explorer and view your downloads. Extract the mod manager to the folder you just created. Double click the folder in your downloads, hit extract all, then browse and select your Mods folder. After double clicking to select the folder hit "Extract" in the bottom right corner of the window.

Open the folder and create a folder inside of it named "Games" and a folder inside that one named "Dragon'sDogma2", and a final folder inside that one named "Mods". This is where you will save your mods in the future. Once a mod is saved to this folder, it will automatically be viewable in the mod launcher and can be toggled on or off with a left click. Now launch the Modmanager application.

Select "Choose game", and choose "Dragon's Dogma 2". That's it! Now you can explore and download mods, save them to the folder you created, and enable/disable any you want. Below is what my mod manager and mods folder look like.

You can see my first "Mods" folder is in a folder named "Stuff" on my desktop. Inside of it are the folders you should have also created named "Games", "DragonsDogma2", and "Mods". This is where I extract all the mods I download. Once downloaded, I can enable or disable any I want in the mods manager.

Next, download the REFramework mod like you did for Fluffy's Mod Manager.
REFramework: https://www.nexusmods.com/dragonsdogma2/mods/8
This is a bit different and we are going to save it somewhere else. Go to your downloads folder and extract it to your Dragon's Dogma 2 game install folder. The file path is:
Local Disk(C:) > Program Files (x86) > Steam > Steamapps > common > Dragons Dogma 2
Just select the folder and the extracted file will move to this folder.

You can now mod your game as much or as little as you like. Have fun!

The Dragon’s Dogma 2 achievement note should be edited to add you have to climb under the dragon as you’re riding in order to use the Godsbane sword. I had to look that up on another thread. I couldn’t use the sword just sitting on its back.

The achievement "An Eye for an Eye" can also be accomplished with the shield called "Daughter of the Evening" found using the Vault Key to loot the castle vault in Verworth. Shield has flavor text of "A shield polished to a mirror sheen. Reflects the faces and baleful powers of foes back upon them, that they might know their own horror." Worked to reflect the Medusa's gaze and petrify it while defending.

I spent over an hour and literal days in-game trying to get "Are We There Yet" after my glitch, and nothing worked. Some claimed sleeping several times etc worked but it didn't reset for me despite everything. NG+ was the only hope there @Lumi.

Some sources say you can but I was unable to obtain "I'm In" after making it past that point on my first playthrough. You can try but I'm not 100% sure if it will work.

Is there a way to complete "I'm in" and "Are we there yet" if you get past that point? Those will probably be the only two I will have to go to ng+ for...

For the Sphinx's "Riddle of Differentiation". Instead of trying to look at their teeth, you can tell the difference with their hair. Vergil has longer wavy hair that parts towards his injured eye, and Dante has shorter straight hair that parts towards his good eye.
