This is the DEATH MASK trap, or also referred to as the VENUS FLY TRAP, Seen in SAW II.
It is one of the first beginning traps of the film


To start the game, you must read what Jigsaw says and he says some pretty big things
Once it is fully read, the machine will begin counting down, to 60 seconds, before the trap closes in and puts spikes through the victim's head.

You must grab a knife, located on the other side of the room, and use it to tear out their eye stitching (the blood marks), to get the key and free yourself.
However, you notice there's no key..

That's about it.
