No need to start new game.
It can work in 182 start too, but if he is dead, in the character pool or if he simply dosen't exist then the mod will not work.
I gave him +60 satisfaction (others with the same skill will have +60 too).

Program Files\Steam\steamapps\common\Total War THREE KINGDOMS\data
put your mod file there with a png picture that has the same name as the mod file (can't be bigger than 1mb). Then you will see your mod in the launcher where you can select your mods which one you wanna use. Your own mod will have a blue folder icon instead of a green check mark. Click on the blue folder icon and upload your mod. After that you can find your mod in your workshop. You can change the privacy there ( friends only, public, private, hidden). If you put it to public, then your mod will appear to everyone.

I did not test it in 190 start date, because I'm lazy as hell, should be okay tho.
