Make your first meaningful choice

This achievement is obtained through natural progression.

A red flower will show up on the top right corner of your screen after making a meaningful choice.

Ball Gown

Look killer on the Grand Ball

After accepting help from The Fairy, you'll be given a dress so you can attend the Ball. The achievement is obtained soon after attending.

** As IllusionedEyes pointed out, it doesn't matter who you go to for help (witch/fairy), you'll still get the achievement.

Grim Skulls

Die a miserable death

This is the path I followed to get the achievement. This is basically the result of failing to poison Carmosa.

Just remember that this is simply one variation in the story so there are a few other paths you can take to obtain this achievement.

The choices:

This achievement is obtained by escaping town when given the choice (by the fairies in my case - see Grim Skull if you want to take the same path to get to that decision point). You still get to attend the palace event except you end up leaving everything behind as the achievement description suggests.

This achievement is obtained by going for the prince when given the choice (by the fairies in my case - see Grim Skull if you want to take the same path to get to that decision point).

Candle of Wisdom + Phial of Venom + Family Necklace

Discover Carmosa's secret

You get this achievement shortly after meeting the Shady Character in town. I marked it down on the choices below to show when it's achieved.

Phial of Venom
Do something really bad

You get this at the end of the choices below. To successfully poison Carmosa, you need to be on good terms with her.

Family Necklace
Gain control of your inheritance

This achievement is obtained for getting the Independent Woman ending. One of the variations for this ending ends with poisoning Carmosa. If you follow this guide, you can get all 3 achievements in this section in one playthrough.

What alexrdavies said, but do note that if you have wooed the Captain, he will save you if you fail to poison the stepmother, so in addition to making sure you're hated by Carmosa and don't know of the family's financial situation that you also have not gotten too close to him.

When you are trying to become the queen, you need to be careful of your decisions. They will determine alternate queen endings (one where you are loved and one where you are hated).
