Head To This Location, Its A GRE Anomaly So I Would Suggest Going During Day Time, Jump Down As Shown Below, And Lock Pick The Car's Trunk And Pick Up The Third Duck.
Head To The Location, There Is A Dark Hallway, In Front It, Follow As The Gif And Pick Up The Fourth Duck.
Fifth And Final Duck Can Be Found In A Watch Tower At Observatory Entrance, Head Over There, And Climb The Truck, Then Follow The Gif Inside The Tower And Pick Up The Last Duck.

Head To The VNC Tower And Get Inside The Elevator, Select Basement And Follow Through The Water And Get To The Other Side, Follow Through The Door And Get To The Ritual Room.

Place The Ducks On The Tables And Use The Cables To Make A Pentagram, Then You Will See The Doom Shotgun In The Middle That You Can Interact With To Start A Challenge.

When You Reach The Third Gif For Dying Force, Simply Turn Left Towards The Red Hallway Instead Of Right, You Will See A Small Box On Right Corner Of The Room With Mistress Sword Blueprint On It.

Simply Un Equip All Your Accessories And Get Inside, Kill Couple Of Infected And Quit To Menu After Like 20s, And Load Back In. You Should Have The Doom Shotgun In Your Accessories.

For This Item, During The Main Quest Called "A Place To Call Home", You Have To Tell Jack Matt What Actually Happened To Aitor.

Later On During The Main Quest "On Board" Aitor Will Be Brought To The Ship Wounded, You Will Receive A Side Quest Called "Aitor" Which You Want To Do As Soon As Possible Before Getting To Main Quest "Broadcast".

When You Reach The End Of The Quest, You Have To Make A Choice Which Determines Weather Aitor Survives Or No, The First Choice Doesn't Really Matter, But As For The Second One, You Have To Go By "I Give Aitor The Small Petals." In Order For Him To Survive.
Then Simply Head To The Fish Eye After Sometime To Receive The Lazarus From Him. (Thanks To Pig_Main For Reminding Me!)
