Intro
So I'm not much of a horror fan, but I bought this game a while ago after being intrigued by the art style (clearly inspired by John Kenn, go look him up!) and decided to finally play this game for spooky October. After my first blind playthrough, I only got a couple of achievements with many still hidden, and I quickly became curious about what they might be, which lead me down a very deep rabbit hole...

This game is a lot more complex that it seems on the surface. There are many different types of runs and endings, with a delicate karma system that dictates how good, bad or neutral you are, and how happy, difficult or nightmare-ish your game becomes. One small action in an early chapter can completely change how a later chapter plays out and each problem has several solutions depending on what you've done and what items you have in that run - all of which can make it very difficult to write a comprehensive guide.

With a quick google, there is a guide on steam that seems to answer all the questions - a complete achievement guide, hurray! But, like the nightmare of the game itself, these links are broken. Not deterred, I did more digging, and managed to find small fragments of information all over the internet - hints buried in comments, small details mentioned off-hand in old forums - and after a LOT of research, I have 100% completion. It was like a frustrating scavenger hunt, trying to piece together what I needed for the complete puzzle when I just couldn't find the solution myself. Even though someone could feasibly go to the length that I went to to do the same, I thought it might be a kind deed to compile all of the information I found into a single comprehensive guide. I've never written a guide before, and I'm sure there will be other solutions not described in this guide, but I'm going to try my best to explain how I completed them and hopefully it will spare someone hours of searching old forums and comments!

Ok, enough rambling from me. Here is the guide, as best as I can explain, and with credits for where I found certain solutions. Thanks for reading, and have a nice dream~

Peaceful Nap, Grim Reaper, Villain and The End
Peaceful Nap (the good ending), Grim Reaper (the neutral ending) and Villain (the bad ending) are all together here because, unlike all the other achievements, they do actually have complete guides on steam, yay! There is no need for me to repeat the information here, so go check out those guides if you need help with any of these. Knowing how the good, neutral and bad routes work will also help with many of the achievements! Many thanks to Convenient Spider for your help!

Good Ending Route (Peaceful Nap) by Convenient Spider

Neutral Ending Route (Grim Reaper) by Convenient Spider

Bad Ending Route (Villain) by Convenient Spider

I've also added The End here, because this achievement is automatically unlocked when you have all three of the endings. I think it's interesting to note though that you can complete the game without getting one of these endings! You karma isn't strictly binary - for example you can be mostly neutral, but do several bad things and get a sort of neutral/bad ending which gives you neither of the ending achievements. This happened to me in my very first play of the game!
Victim
This achievement is actually achievable if you follow the bad end guide by Convenient Spider (see the Villain achievement), but I'll list it separately because there is a split ending so I want to make it it clear what exactly triggers the different achievements! Credit to them for the solution :)

Play through the bad route as shown in the guide, all the way until the last chapter. Play through the chapter as the guide says, until you come to the cyclops. Instead of defeating the cyclops, you must do NOTHING and let him destroy the hospital. This will give you the Victim achievement.

Killing the cyclops will get you the Villain achievement instead!

It seems that this achievement can be unlocked by other ways of dying (such as from bleeding out), but this method is what got me the achievement!
Full of Life
For the Full of Life achievement, you must take absolutely no damage. I think this is quite an easy one to get when you do a perfect good run, I got it without trying while following Convenient Spider's good end guide.

Main notes are avoid doing bad things, use the gardening glove to pick up spiny flowers and leeches, don't touch mice spiders or birds, don't touch the scythe in the crypt, don't touch the toys in the old house, use the crowbar and gardening gloves to open the ? box in the old house, use the crowbar to trigger the mouse trap, don't touch broken glass, and so on.
Game Modder
Another simple one, I don't think the karma matters until this point but doing it will give you bad karma. When you get to the apartment and play the board game, use the pen to scribble out the enemies and hazards until you get the achievement.
Upper Floor
This one is slightly more tricky and many people miss it in their runs. There are several way to do this - I've done it two ways, and got two different scenarios in the upper floor of the hospital in chapter 2.

The first way is to complete the first chapter without killing the birds, but smash the doll for the wire - this makes you neutral. Then once you are at the hospital, kill the spiders. This will make spiders hate you. For the rest of the hospital, try to not hurt anyone. This should trigger the doctor to ask you about the second floor at the end of the level. Once you go up, there will be a giant spider who won't let you back down unless you get her babies back! You have to get the glove from the hole in the wall, and then get the key from the green slime. Then unlock the locked room and use the wooden stick on the floor to poke the box - here are her babies! Leave the room and bang on the door with no handle, more babies will crawl out. Go back to the giant spider and the doctor will appear in the elevator. A true nightmare for arachnophobes like myself!!

The second way is to complete the first chapter the same way - ignore the birds, smash the doll. Then, at the hospital, don't hurt the spiders. Instead, turn on the shower, and poke the pervert's eye out with the scissors. Also make sure to flush the toilet and receive the doctor's room key - DON'T USE THE KEY. Complete the rest of the level without hurting anyone, and the doctor should ask you about the upper floor at the end of the level. In this version, you didn't hurt the spiders, so there is no giant spider. Instead, the elevator button is just broken. Use the doctor's room key to open one of the rooms. Inside there will be a hole in the wall - cut the wire and get the gloves. Use to gloves to pick the key out of the green slime, and then use the key for the locked door. In that room is a screwdriver, which you can use to fix the button!

There are probably more ways to complete this achievement, but these are the two ways I experienced. The trick seems to be to finish chapter 2 as a mostly neutral/slightly bad person. It was thanks to information on this forum[forums.bigfishgames.com] that I could complete this achievement.

I recommend pairing this achievement with Camera, providing that you did the second solution where you poke the pervert's eye out. This isn't the only way to do it, but in my experience it immediately leads into the Camera achievement!
Camera
EDIT: There seems to be some debate in the comments on how to get this achievement! What I've detailed below is how I remember getting the achievement, but for some reason this doesn't work for everyone - no idea why! Perhaps I misunderstood which achievement I got, or maybe it was a glitch, who knows. I've also included a different method that seems to work for other people underneath.

Follow the guide I wrote for the Upper Floor achievement, making sure to take the route where you DO NOT kill the spiders. Complete the chapter and continue to the graveyard. Go straight to the woman sitting on the bench and talk to her until she gives you the camera! That should do it.

You can use the camera to avoid damage when the Gravedigger attacks you in the Graveyard, but this is entirely unrelated to acquiring the Camera achievement and does nothing to trigger it.

ALT method below, huge thanks to Lunessence for detailing it:

Ended up getting the 'Camera' achievement by following the bad ending route. When you start Chapter 8, instead of going back to the graveyard right away, click on the ladder that leads back up to the house and click on the doorway in the picture to leave the parent's bedroom. There is a battery in the toy car in the child's room and film near the TV on the floor in the living room. Then, go back down to the basement and enter the graveyard by clicking on the tombstone in the drawing on Death's table. Talk to the kid. He disappears. Cross the bridge on the right and enter the house with the key. There should be a camera sitting on the coffee table. Put the battery and film in the camera. Pick up the camera. The achievement should then pop up.

Hopefully one of these methods works!
Fishing Master
This one is fairly simple but easy to miss. You must play the first chapter completely good - don't hurt the crows, break the controller for the wire instead of the doll. When you get to the end of this chapter, the homeless man asks for change. Give him the coin, BUT DON'T MOVE FORWARDS! Instead, go backwards a couple of places and there will be a fishing rod hanging over the edge of the bridge. Click it and you will find the special bait. Continue as normal (karma doesn't matter, but good would be easiest I think) until you get to the fisherman in the forest. When he asks for bait, give him the special bait. Done!

I recommend pairing this achievement with Secret Room, Special Treat and Clean Mirror, since these also require a perfect good run!
Secret Room
For this achievement, you'll need to go through the game perfectly good and get to the apartment. In the apartment, go through the puzzles as usual until you get the paperclip. Instead of unlocking the locked room in the mirror apartment (the one you're currently in), go through the mirror and unlock the locked room in the other apartment. This is why being good is necessary, if you're neutral or bad the mirror will get smashed and you won't be able to bring the paperclip to the other apartment!

I highly recommend pairing this achievement with Special Treat, Fishing Master and Clean Mirror, as they all require a good run!

Thank you to the nemsei for posting a video showing how to do this!
Special Treat
For this achievement, you must do a perfectly good run. When you get to the apartment go through the level as usual, but go to the secret room (see the Secret Room achievement) and find the sausage on the table. Carry on with the good run with the sausage, right through to the last chapter. As you go through the last chapter, the dog will appear in the old house forest, blocking your path. Usually you give the dog the ball you find in the tent, but for this achievement give the dog the sausage. Good dog!

I highly recommend pairing this achievement with Secret Room, Fishing Master and Clean Mirror as they all require a good run!

Thank you to the nemsei for posting a video showing how to do this!
Clean Mirror
For this achievement, you need to go through the whole game perfectly good. When you get to the old house, continue as normal until you find the dirty rag. In a neutral run the rag is bloody and can't be cleaned, and in a bad run the solution is very different (you have to get the rag from the radiator inside, clean the picture and cut the eyes out of the drawing!) so being good is very important.

Take the dirty rag to the lake in front of the house and you will be able to clean it. Continue as normal, but BEFORE YOU ENTER THE BASEMENT MAKE SURE TO PICK UP THE RAG AGAIN. Take it with you to the apartment and go through as usual. When you are tasked with cleaning the mirror, use the soap spray but DON'T use the toilet paper - use the clean rag instead for the achievement!

If you forget to clean the rag in the lake, you can apparently clean the rag in the apartment sink, but I've never tried this myself.

I highly recommend pairing this achievement with Fishing Master, Secret Room and Special Treat, as they all require a good run!

There is a hint to this solution in this thread[forums.bigfishgames.com], but I had to piece it together myself!
Call Me
The clue to this achievement is hidden in the Secret Room (see the Secret Room achievement). In the room a piece of paper has the number 637-511 written on it. In a NEW, perfectly good run, get to the hospital in chapter 2 and dial this number. You will call the phone in the Secret Room. It must be a good run otherwise the phone will be broken!

I highly recommend pairing this achievement with Warrior, as you need the shield from the Secret Room board game to complete the Warrior achievement!
Warrior
For this achievement, the goal is to use the shield in the board game! But what does that mean? By complete luck, someone mentions the solution written in the deleted guide in this thread! I'll also explain here to make things simple.

Firstly, you need to play the game perfectly good. I'm not sure if it is completely necessary after chapter 2, but it will make things easier to stick with good. In chapter 2, ring the Secret Room phone number 637-511 (see Call Me achievement for more details!). While in there, play the board game and get the shield.

Continue with the chapters as usual, until you get to the board game in the apartment. While playing, don't cheat and get to the final boss. You then need to land on one of the bad spots (the red circles) at the boss monster - you will automatically use the shield and not get sent back to the checkpoint, unlocking the achievement!
Master Key
According to the description of this achievement, at one point it was a bug :P

You must smash the doll for the wire in the first chapter, but otherwise play good. As you go through the hospital in chapter 2, you will need to flush the toilet in the bathroom to get some items: a worm to get the eye from the fish, and the key to the doctor's room on the roof. DO NOT USE THE KEY! You also get the bottle cap from the floor and the hair from the sink. The key to the bathroom is in the cafeteria. Trade the bottle cap with the bird and give the worm to the fish for the eye. Place everything on the patient in room 15, then go back to the window where the bird was and collect the other eye from its nest.

Finish the level and continue through to the forest. Use the doctor's key on the cottage in the woods for the achievement!

Thank you to hurtybitey for detailing the solution in this thread!!
Tailor
This achievement is a little tricky, as it requires a specific set up. In the first chapter, do not kill any birds, smash the doll to get the wire, and continue normally.

Then at the hospital in chapter 2, DO NOT PICK UP the scissors before fixing the patient who needs eyes, ears and hair. To do this, you grab the hair from the sink in the bathroom, and flush the toilet to grab the worm on the floor. Grab the bottle cap from the bathroom floor as well. Then go to the window and give the bottle cap to the bird for the ear. Go to the computer room and put the worm in the bowl to get first eye. After you put the eye, the ear and the hair on the patient, go back to the window and grab the eye from the bird's nest. Put the eye on the patient. After the dialogue, the doctor will have you take the bus to go to the graveyard BUT BEFORE YOU LEAVE, go back to the computer room and grab the scissors and matches in the drawer under the fish bowl. If you have them in your inventory before talking to the doctor, the doctor will take your scissors so it is important to do it in this order.

Proceed through the next chapters normally. When you get to the apartment, you should still have your scissors. When you go through the mirror to the second apartment, use the scissors on the coat that's hanging by the mirror and you'll get the achievement!

Big thank you to Koko for detailing this achievement in this thread!
Voice of Reason
While I'm a little unclear on what the real goal is for this achievement (it seems to involve avoiding damage as much as you can, and having the kid accompany you at the end), there is a decent guide for the important steps here written by Karosty (I think informed by the deleted guide?) with additions by MASTAN - thanks to you both!

The information is a little spotty and scattered, so here is exactly what I did that got me the achievement in the end.

In the first chapter, I killed all the crows but did not smash the doll, getting the wire from the controller instead. As the bird monster chased me I went backwards for the eyeball.

In the second chapter, I cut off the fingers of the man in the full body cast, I did NOT kill the crow on the windowsill (traded for the bottle cap instead), and did NOT kill the spiders. Also, I did not stab the pervert's eye in the bathroom. I get the other eye from the fishbowl instead.

In the third chapter, I go to the woman sitting on the bench and talk to her until she gives me the camera (this is an alternate way to unlock the Camera achievement if you haven't already gotten it!). I avoided taking damage by using the camera on the gravedigger when he attacks, and using the glove on the spiny flower. Complete the rest of the chapter as usual.

In the fourth chapter, complete as usual. I'm not sure if it matters, but in this run I fed the dog the meat rather than killing the dog with the beartrap. Make sure to wear the glove to avoid damage from the spiny flower and leech.

In the fifth chapter, complete as usual. The butcher appears to kill the man in the bathroom, and it seems there is no way to avoid him hurting you as you get the drink for the old man.

In the sixth chapter, complete as usual.

In the seventh chapter, complete as usual. Be careful to open the ? box with the crowbar and take the scissors with the gloves.

In the eighth chapter, this is where it gets different! I talk to the kid, then go to the house over the bridge to get the skull. Put the skull on the crypt and trigger the hot air balloon to the hospital. Then the kid asks me about the camera - I got the film from the TV in the kitchen, and the battery from the radio in the cleaning room. Then I took pictures of the poisoned camper by the vending machine, the butchered man in the bathroom, the man with the guts hanging out and the man in the full body cast (I'm unsure if which people I photographed mattered - in the end, Death mentions that I hurt some of the people in the photographs, so it may be important. I hurt the poisoned camper and the man in the full body cast, but that was it, the other two photos were of random hurt people. If it matters, 2/4 people hurt seemed to work!).

Once I was done with that, the kid disappears. I walked around a while, seeing the butcher appear in various places, as well as the dinner lady's head and the bathroom key in the kitchen. I pick up the key, go in and out of the bathroom, then up to the doctor's room at the roof. This is where the butcher should chase you, and you're supposed to lock him in the bathroom - but for this run, I let him catch me. Afterwards the kid came with me and picked up the head of the dinner lady. I returned to Death in the crypt, and it seems as though Death is about to kill you like in the Villain ending, but the kid intervenes and is the "voice of reason" in the end. Achievement unlocked!
Dirty Cursor
This is one of the hardest achievements in the whole game, so much that it seems the original guide makers were given a hint from the devs to help figure it out! The goal of the achievement is to see a reflection of your cursor when it is very dirty. If you've played the neutral ending already, you might remember seeing your cursor close up in the mirror right at the end, but clean. So... how to make it dirty?

Luckily, there is a VERY helpful guide for us, written by Sorathil. This guide is very good and comprehensive and deserves a huge thank you, but there are some notes I would like to add where I personally struggled because it didn't explain some of the details of this very specific run!

Firstly, make sure to pick up the eyeball in the first chapter. When you get attacked by the bird monster on the bridge, quickly click backwards a couple of spaces. You will come across birds eating flesh, and can pick up an eyeball there before losing your fingers. This is mentioned by the comment left by Mon-Tee on the guide, so thanks to them for pointing it out!

Also, DO NOT FORGET THE COIN! It is very important in the next chapter, and I totally forgot it in my first run of this and got stuck.

In chapter 2, there is a very unusual set up that makes things tricky. In a bad run, you lose your fingers but can just chop them off of the guy in the full body cast. In a good run, you don't lose your fingers so you don't have to hurt anyone to get them back. But, in this particular run, you have lost your fingers, and then have to get through the hospital without hurting the man in the full body cast.

So how do you get your fingers back without hurting anyone? Capture the spiders for the dinner lady, and then speak to her in the kitchen. She will ask you if you want to buy soup. Giver her the coin and you will get a bowl of soup with fingers floating in it! From there, you can continue chapter 2 as you would in a good run; trading the ear for the bottle cap, the worm for the eye, etc. (hurting the fish with the worm doesn't seem to count as injuring something!)
Huge thank you to hurtybitey who by chance mentioned this soup solution in a comment, they are the only person I've ever seen mention this way of solving the chapter, and I'm not sure I would have figured this out without them!

In chapter 8, when you get to the crypt you can look at the picture and click the hot air balloon BEFORE clicking the mess on the floor and getting your cursor dirty. I'm not sure if this actually helps, but I feel like it minimises the clicking you do once your cursor is dirty (the more you click, the more clean your cursor gets!). Also, while talking to people, click through the dialogue with your cursor on your inventory items instead of just a random spot on the floor to minimise accidental clicks (again, thanks to Mon-Tee for this tip!).

Aside from these notes, Sorathil's guide is very good and following it with this extra information in mind should get you this very tricky achievement!
Helpful Butcher and Messenger
These ones are listed last and together, because I randomly happened to get these two on my first playthrough and I am not 100% sure what triggered them, or if one is necessary for the other. Lucky for my completion, but unlucky for this guide :(

From what I've pieced together, the Helpful Butcher requires the butcher to be in the hospital - he gives you a "gift", an eyeball in the bathroom sink you can use to help the patient in room 15. Getting the butcher to appear is a little specific, but otherwise it's not too hard.

Messenger on the other hand, I have yet to come across anyone who can explain exactly what the goal is, or what steps are necessary. It seems as though there are many ways to achieve it. My personal theory is that Messenger is triggered when you have a neutral/bad run, and get none of the "true" endings (Villain/Victim, Grim Reaper or Peaceful Nap). I also think the butcher needs to be active, and the kid must also not be with you for the end, otherwise I think it will trigger the Voice of Reason ending. In this ending, you give Death the photos of people suffering, and she goes back to her job without hurting you or making you take the job instead - so being a "messenger" would make sense in this situation.

I'll do my best to go through the steps I went through in that first run to the best of my memory. I'm not sure if every step is entirely necessary but hopefully it will be enough to get both of the achievements the same way I did!

In the first chapter, kill every crow, and smash the doll. When the bird monster appears on the bridge, DO NOT GO BACK FOR THE EYEBALL. Just let the monster eat your fingers and move on to the next chapter!

In the hospital, grab the matches and scissors and cut off the fingers of the patient in the full body cast. Then talk to the dinner lady and kill the spiders for her, and get the glue out of the cleaning room to fix up your hand and grab the worm. Kill the bird on the window with the scissors, take the ear, and use the worm on the fish for the eye. In the bathroom, DO NOT POKE OUT THE PERVERT'S EYE. Instead look in the mirror, and grab the hair from the sink. Put the ear, hair and eyeball on the patient in room 15, then come back to the room with the man in the full body cast. He should have his eyes and ears cut off, and the bed cards should have been edited with blood - this means the butcher is here and we are on the right track! Go back to the bathroom and check the sink again - an eyeball should appear as well as a message from the butcher - and you should get the Helpful Butcher achievement! Finish the level and continue to chapter 3.

In the cemetery, the scarecrow monster should make an appearance. Click all the birds, the scythe and the giant spider for damage. I also didn't use the glove to handle the spiny flower. Instead of getting the piece of rotten flesh from the spider, get it from the scarecrow when he kills the zombie. After talking with Teddy in the shed, the scarecrow attacks you outside - erase it at the gate entrance. Next chapter!

In the forest, feed the dog the meat then continue as usual, poisoning the camper. Take the spiny flower and the leech without the glove, and feed the leech your blood by hovering the cursor over it while it is on screen. Finish the chapter.

In the next chapter, the dinner lady will ask for the key to the gate, and the Nurse will be stuck under the manhole cover. Free the Nurse and give the dinner lady the key (she will be killed by the butcher behind the gate). Then go back to the hospital and give the guts guy the 13 ticket. Click the leech in the Doctor's Room on the roof, and the ones in the bathroom too. I also gave the old man toilet water (the butcher will attack you during this puzzle). Try to give Teddy the scary clown toy and the pills before giving him the nice picture. Next chapter!

In the apartment, go through as usual. I touched the broken mirror pieces for more damage. When you play the board game DON'T use the pen, but do dishonestly reroll the die. Next chapter!

At the old house, kill the weird rabbit and use its leg to scribble over the GO AWAY sign. Use the pen for the second sign. Everything else plays out as usual. I touched all of the toys for more damage. I also hurt myself opening the ? box and taking the scissors (my nose!!). Continue on to the final chapter.

In the last chapter, go to the house over the bridge, grab the dead bird and the teddy head, and then get your arm bitten off by the giant spider! Go back to the shed and the Doctor is there. He notices that you have no hands left, and later on in the cemetery he gives you a zombie hand. Move on to the hospital, get the camera, and take pictures of the woman on the roof, the man in the bathroom, the man with his guts out, and lastly the man holding the jar in room 15 (I think it was totally accidental at the time, but looking now I think none of these people are people I have hurt myself, so that might be important!). The kid goes off with the film, and the butcher starts to appear. Once he's been in the kitchen, grab the bathroom key, go in and out of the bathroom and then up to the roof. The butcher will now chase you! Go back to the bathroom, wait for the butcher to enter and then step out and lock the door again, trapping the butcher. After you lock the butcher in the bathroom, quickly go back to the roof and talk to the kid so he can give you the photos, take the hot air balloon back to the cemetery and go back to death to give her the pictures.

Big thanks to Vaagur for testing this and correcting a detail for me! According to them this method should be successful now :)
Final Notes
I hope this guide is useful and helps people complete this interesting and complex game! I didn't think I would enjoy it as much as I did, but the surprising depth of the mechanics and the changing story really hooked me! As I said in the Intro, it would be almost impossible to fully detail every possible route because there are just too many branches, but I'd like to point out some of the interesting observations I noticed in the various runs, as well as some extra hints for parts that took me a while to figure out. Thanks again to all the people who helped me figure out the tricky achievements, and thanks to you for reading :)

  • The game in general seems to get more difficult and gruesome and filthy the more you do bad actions, and more monsters will chase and attack you. In many cases as well, doing bad things early on forces you to do more bad things later on - such as poisoning the camper. The opposite can be said for the good route, as everything gets cleaner and you are left with more good/neutral puzzle solutions. This is reflected in the story too - the characters explain that the more scared and unhappy the people are, the more nightmare-ish the world becomes. If you stab the eye in the shower, you're put on the path of the cyclops - the cyclops exists because you created it. If you kill the spiders for the dinner lady it feeds into her fear more, and later she is terrified because she creates even bigger spiders. If you don't kill them, she realises there's nothing to fear and she never has to run from those giant spiders. Like the woman on the bench says: it's a vicious cycle. This isn't super relevant to anything, I just think it's neat.
  • In chapter 1, there are many different variants of the "spot the difference" puzzle the homeless man gives you. If you kill any birds a dead bird shows up on the first puzzle. There's also two versions of the first puzzle, one with more differences, depending on how bad or neutral you are. The second puzzle is also harder the more bad you are - if you're neutral, the second puzzle is the trunk of the car, but if you're bad the second puzzle is a dead bird leg which is pretty tricky (it took me ages to realise that the tear in the bottom right of the paper is one of the differences!)
  • There are three variations of the drink you feed to the camper in the forest. An "energy drink" for good, "drug" for neutral and "poison" for bad. The more bad you are, the more ingredients you have to find. If you have trouble reaching the egg for the poison, you can find the ladder pegs in the cottage (partially hidden behind the fireplace) and on the pier where you find the leech.
  • The board game also gets harder the more bad you are - if you trigger monsters like the scarecrow or the butcher, they will appear on the board as extra hazards to avoid. Also, in bad runs the doorbell will ring while you're playing - if you answer the door, the bully appears and adds even more hazards to the board game.
  • The game seems to change as you harm certain creatures in the game. If you kill enough spiders, they crawl all over your screen, appear in the Upper Floor and harm you in the last chapter. If you kill enough birds, there are a lot more birds and the scarecrow monster will appear in the cemetery (apparently the bird monster from the bridge makes a second appearance sometimes too, though I've never managed to trigger this!). If you hurt the dog with the bear trap, the hospital will be infested with mice and there will be a mousetrap that will chop off your fingers if you don't use the crowbar!
  • There are a few doors that never open in the game, despite having padlock symbols on them that imply they could be opened. The ones I have encountered are the basement door in the apartment, and the door inside of the house you find when you return to the cemetery in the last chapter. I've also noticed that when you cure the computer, you can click the clock to change the time, but I haven't noticed any changes when I've fiddled with it. Very curious!
  • There is a funny easter egg in chapter 2. Turn the light off to change the hospital to night time, and then go to the roof. Use the magnifying glass on the moon to see a UFO!
