TTT In Game Karma Ban

normally, if a player's karma gets too low, they are banned off the server or at least kicked off.
this makes karma bans into an in game punishment, where a player is autoslain on round start.

when a player is autoslain, they are killed with explosion damage, an explosion goes off at their feet, their corpse is turned black and shot up into the air, and the player is also marked as dead on the scoreboard.

ulx commands:
ulx karmaban <players> - add a karma ban (say: !karmaban)
ulx karmabanid <steamid> - add a karma ban on a steamid (say: !karmabanid)
ulx karmaunban <players> - undo a karma ban (say: !karmaban)
ulx karmaunbanid <steamid> - undo a karma ban on a steamid (say: !karmabanid)

convars to note:
ttt_karma_low_amount when a player's karma reaches this, they will get karma banned.
ttt_karma_low_ban if set to 0, the autoslay will only kill for 1 preptime and 1 round, otherwise they will be autoslain for the full duration of the karma ban.
ttt_karma_low_ban_minutes duration of a karma ban, will only work if ttt_karma_low_ban is set to 1.
