Pain elemental from Doom II by id Software.

The Pain elemental is a floating sphere with a single eye and a large toothy mouth. Its single eyeball is red color with a black round pupil, its hide is leathery brown, and it is crowned by two dark curved horns on the top of its spherical body. Instead of shooting a standard projectile, it spews lost souls at its opponents from a mouth that flares up with a reddish glow each time one of the minions is delivered.
