Secret Eggs/Costumes
In each world, there is a level with a small collection of eggs somewhere (Pictured is a part of 1-9):


By reaching these eggs, you permanently unlock a skin. You can change skins any time via Main Menu -> Options -> Extras.

The levels with a question mark are those whose locations I'm not sure about -- Once these eggs are collected, they disappear from all future playthroughs of the level, unless you clear your save file, so it takes a lot more effort to double-check these levels than I'm willing to bother with.
Secret Sequences
It's a mystery to me, too. I think the only relevant inputs are level manipulations (Rotate left, rotate right, flip).

This seems to be an achievement involving dropping blocks out of levels, but I haven't been able to figure out what exactly this is looking for.

1) I can confirm that the Wily Woods secret eggs are on 2-4.
2) I can confirm that The Lab's secret eggs are on 4-9.
3) I haven't figured out any sequences, but I notice that the ALLCAPS names of the secret sequences are all species of kiwi birds [en.wikipedia.org] Not sure if that's meant to be a hint (I doubt it).
4) You don't mention the "Uh oh" achievement, but I unlocked it on 3-11 -- although I'm not sure if it triggered as intended.
5) The "I'm So Sorry" Achievement can only be unlocked on 4-18, as far as I can tell.
