I'm a Source mapper / level designer that works in both Portal 2 and Garry's Mod. My goal is to create maps with unique environments and concepts.

By the way, I have figured out how to fix the AI nodes, but I have been doing other things for the last couple days. I will fix both versions soon.

The nodegraphs in your maps are kind of.. broken. Like they don't fully connect and sometimes certain NPCs won't run to you.

I use gmad.exe (in the GMod bin folder, with Hammer and other things) to convert to the .gma, and then gmpublish.exe to upload my addons. I don't know how other third party tools work, but if you follow the GMod workshop guide and then go through the steps below, the icon should work. If you want to test your icon, subscribe to your map and then open GMod and look for it again in the "Start New Game" menu. If it's working correctly, you will see the icon just like anyone else would.

First, if you don't already have one, create a folder named "thumb" inside the maps folder of your addon. Create a .png file that is 128x128 pixels and name it the exact same name as the .bsp file. For example, if I have needsicon.bsp, then I would name the image needsicon.png. Then put that .png inside the maps\thumb folder of your addon.
