Why do you have a game files on your desktop? That isn't a suitable location.
May I suggest moving it to a more suitable location e.g. C:/Games/Iw4x

Emblem / title data is stored in your stats file, your stats file is saved to the players folder in your game folder. The only reason your title and emblem choice isn't saving would be because the game can't save your stats because it doesn't have perms to do so.
Is your MW2 game folder by chance in program files folder?

I just downloaded iw4x too and I noticed this too, I was worried but I hope it will be fine

I think I found a way to avoid this. If you reinstall the game, and then never open MW2 multiplayer from steam, avoiding the first time setup and just launch from the iw4x app, I don't think Spacewar shows up

I believe the code just checks if it's in your library aka purchased.

These are the ramblings of an idiot or a mad man. His videos just make him seem foolish for pretending to know more than he actually does. I tend to avoid the Steam forums as there are many of these such people who just circle jerk each other.

You will need to either reset your config file (.cfg) or use the console to re-enable hitmarkers. I am unsure what the dvar is called so your best bet would be asking in the IW4x support channel or check the IW4x website for what it is.

You have to edit the the games code +reverse engineer) but it wouldn't be advised due to stability issues.

Basically you cannot do it via dvars or gsc.

The players folder contains all user data, you can just have two copies of these and just rename them when you wish to switch profile.
