Base game + expansion that's as long as at least a third of the main campaign + arrange mode, which is base game again, but spicier and worth a replay + Angel of Death difficulty, which is a surprisingly engaging implementation of ye olde Nightmare difficulty shenanigans.
Yes they added in a ton of levels, there's tons of weapons, improved a lot of things across the board, improved some visuals.
Except they oddly nerfed the 10 portable medkits from 10 to I think 4. That's the only downside I'm aware of.
Personally, it's not my cup of tea. The enemies, levels, puzzles and weapons don't look interesting and, Shelly's one liners don't land all the time. I rather play Duke 3D, Quake 1 & 2 again.
They added in a silent protagonist option long time ago.
With Aftershock they added a new campaign and an Arrange Mode for the base/original one.
